I had to redo the scenario twice so I was able to correct what I did wrong, but what I did wrong the first time that I needed to correct was I did not calculate the rate correctly of the tube feeding, I did it backwards so I need to ensure I know how to do my dosage calculations correctly and I also need to read my chart more thoroughly because I thought it said I needed to get an x-ray for tube placement and got an unnecessary chest x-ray on the patient the first time. The patient is at risk for malnutrition based upon his BMI being below normal, so we are concerned about the patient gaining weight rapidly as well as managing his CF. He is also at risk of an electrolyte imbalance; his creatinine was low, his albumin was low, and he has a vitamin D deficiency so we are concerned to raise his laboratory values and bring them back to normal. We are also concerned about patient education of his diet of a high-fat, high-calorie diet to ensure he does not return for the same issue and potentially putting himself at more risk for complications of his CF. Vitamin D is important to strengthen your bones so with a vitamin D deficiency he is at risk for a loss in bone density which could result in osteoporosis, making him more susceptible to bone fractures.
