Mod 4 Discussion Transition of Care Case Analysis
Henry’s strengths include being able to assist his wife with her care
needs, he is proud of his career as an engineer for the rail system. He is proud
of his late son, his daughter-in-law, and his grandson. Him and his wife attend
the Baptist church. He is realizing that him and his wife will need some
assistance when they get back home, while they are waiting for the assisted
living apartment.
Henry’s vulnerabilities include being the only caretaker for his wife causes extra
stress on him. He cannot leave her unattended due to her forgetfulness. He also
has COPD, and high blood pressure that he is trying to cope with. He states he is
always anxious and worried about his wife. He also expresses uncertainty if his
insurance will pay for his oxygen and new medications when he gets home.
