Sepsis Case Study A twenty five years old Caucasian woman is driven by her roommate into the care facility for an urgent treatment of a suspected foot infection. The patient and the roommate had beach stroll over the weekend and the patient had a little cut on the right leg as they waded bare foot. Unfortunately, the patient did not realize the injury and it only became evident as she removed tar from the bottom of the foot. She brushed it off but two days later, the foot became too painful to walk. It also started oozing a a thick yellow fluid from the injured part. The triage nurse immediately took the vital signs and recorded as follows pain scale of 9/10, BP 86/40, respiratory rate of 24, heart rate of 130and a temperature of 101.5F. On further HTT assessment, the nurses reported that the patient was oriented to place and person only and arousable to voice. She reported to the triage nurse that she had taken Tylenol that morning for fever and pain relieve. The skin is hot, diaphoretic and pale. The triage nurse calls an ambulance to take the patient to the state hospital for treatment of suspected sepsis case. Upon arrival, the patient’s blood pressure has fallen to 80/40 and she is still tachycardic. The respiratory rate has risen to 30. As the medics transported the patient, they inserted a twenty gauge peripheral IVs in her left antecubital. The patient is hypotential and the medics infuse 500 ml normal saline to manage the condition. They also administer oxygen at 4 L/min. the medics insert a second 20 gauge peripheral IV in the right antecubital, draw blood culture and take a swab sample of the cut and submit it to the lab for culture and sensitivity test. The medics diagnose the patient with septic shock and since the she is still hypotensive, they infuse 30mL/Kg of normal saline. What diagnostic tests fit the patient? The health care professionals ordered a combination of test and worrisome signs including intense pain, hot, diaphoretic and pale skin, very high heart rate and low blood pressure and presence of infection.
